Does an adverb describe just a verb or a verb phrase?
Example, in the sentence 'Foolishly, he decided to take a shortcut through the woods' - Does 'foolishly' refer to the whole verb phrase or just the verb 'decide'?

Adverbs can indeed describe a whole verb phrase. Your example is a case in point; what was foolish was not the decision, but the [decision to take a shortcut].

It refers to the whole sentence. It's adding information to the whole sentence.

It applies only to the verb, but there are adverbial phrases as well as verbal ones.
