You seem to have been neglecting your listening comprehension. You need to expose yourself to more listening options.

Depending on where you live, you might try www.imagen.com.mx and look for living in Mexico a radio show hosted by Ana María Salazar, it has news about Mexico in English. You can listen to it on the website or download as an mp3 file. It is pretty good. This program is produced in Mexico City and the accents are many, of course it is that of Spanish speaking individuals, but a good listening exercise.

Listen to radio, www.accuradio.com select the Swinging Pop Standards, listen to Frank Sinatra, Nat King Cole and Diana Krall, among other singers that have excellent diction and pronunciation, neutral American accent, excellent.

http://www.bbc.co.uk/worldservice/learningenglish/ also features plenty of listening material in British English. Plus, you can listen to it as many times as you want, and some of the audio files also have a script that you can read as you listen.

English is spoken as a second language all over the world and it has different accents, because every persons brings a little of their native language into it. There is no such thing as having to speak English like an American, as long as you are able to express yourself clearly there is no need to adopt an accent that is not natural to you.

Five years is a long time to study English and not understand it. You can also buy entire seasons of popular shows like Lost, Sex and the City, 24, Friends, The West Wing, The X Files, and others, on dvd, which you can listen to again and again, you need to listen more and avoid using subtitles, this is your ear and listening comprehension that need training. The more you listen the more you will understand. The slang is probably what you could have trouble with, and for that go to
www.urbandictionary.com this page features definitions to several words you will hear on some programs, which are not listed in a regular dictionary, I warn you that since this is an open dictionary and not regulated by any institution, the definitions are sometimes explicit or obscene, so discretion is advised.

Buy an English-English dictionary and a thesaurus (this lists synonyms and antonyms) Webster's and Roget's are preferred choices.

chat less and listen more

Good Luck

Are you sure you don't have a hearing problem?
Other than that, being able to understand is purely a matter of listening a lot and getting your ears used to the individual twangs.

Do you practise speaking and listening? Do you know someone to talk to in english? Listen only to english radio on the Web. Just keep listening a lot.
